Your EID and insurance card.
Physician referral letter if any.
Previous medical and radiology reports; CT or MRI CDs and list of medicines you are on.
Tests Results
Tests results are communicated to you in a follow-up visit for which you have to make an appointment. For obvious reasons and in your interest, most of the results in Neurology require to be explained in the clinic, not on the phone as further investigations or treatment discussion is likely to follow.
You can get a copy of all tests you have done under my care, a medical release request form need to be signed. A result can be mailed once explained to you.
Prescriptions
Please be aware a neurologist is not a psychiatrist. Therefore, refill psychiatric drugs cannot be done.
Prescription refills need to be requested three working days ahead. For an already known patient, a refill without visit depends on current health status, you might be required to be seen again.
Controlled drugs prescriptions are subject to a strict regulation by the DHA. Your neurologist needs to agree with the previous diagnosis and treatment in order to proceed with a controlled drug prescription. A previous medical report is helpful if no previous visit in any Mediclinic facility.
If you need a medical report; please allow three working days.
